A toothache that keeps you awake is your body telling you the nerve of the tooth is in trouble. Here is what to do tonight, and when to message us.
Why a toothache gets worse at night
Many patients notice that the pain gets stronger when they lie down. When you are horizontal, more blood reaches the head and the pressure inside an inflamed tooth rises. That is why a tooth that was bothering you during the day can become unbearable after midnight.
The most common causes of severe tooth pain are a deep cavity that reached the nerve, a cracked tooth, an infection at the tip of the root and, in the back of the mouth, a wisdom tooth coming in. Each one is treated differently, so the first step is a proper diagnosis with an X-ray.
What you can do tonight
Rinse gently with warm salt water to clean the area. Take the pain reliever you normally use, following the label. Sleep with your head slightly raised and put a cold compress on the cheek for fifteen minutes at a time. Avoid very hot, very cold or sweet food on that side.
What not to do: do not place aspirin or any tablet directly on the gum, because it burns the tissue. Do not apply heat to the face, and do not take antibiotics left over from another treatment without a prescription.
When a toothache is an emergency
Message us the same day if the pain does not let you sleep, if it throbs with your heartbeat, if it lasts more than a day or two, or if you notice swelling or a bad taste in your mouth. Throbbing pain that suddenly stops can also mean the nerve has died, and the infection can continue silently.
Dr. Moisés Lukowiecki is an endodontist: relieving this kind of pain is his specialty. In many cases the first visit already removes the pressure inside the tooth, and the patient leaves the chair without the pain they came in with.

What our patients say

I had an emergency on a Sunday and couldn’t find a single dentist in Panama to see me. Dr. Moisés Lukowiecki was the only one who called me back. He saw me right away, did the root canal that same day, and the severe pain was gone.

Frequently asked questions
Rinse with warm salt water, take your usual pain reliever as directed and use a cold compress. These steps help for a few hours, but only treating the cause stops the pain.
Sometimes the pain stops because the nerve has died, not because the problem is solved. The infection can continue and come back as an abscess.
Not always. It depends on whether the nerve can recover. Dr. Lukowiecki confirms it with an exam and an X-ray before recommending any treatment.
If the pain is severe, throbbing or comes with swelling, do not wait. Send us a WhatsApp message with your symptoms and we tell you how soon you should be seen.
Yes. Dr. Lukowiecki has treated many children and teenagers with tooth pain, and the clinic also has pediatric dentists on the same team.
